Services may be provided in this health care facility by the facility as well as by other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organizations as the facility.

Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider. If you have insurance, we will file that for you; however, we will expect payment for co-pays, co-insurance and deductibles at the time of service. We accept cash, checks, MasterCard, Visa and Discover. We also offer the option to pay bills online. Pay online using our secure server here. You will receive a separate bill from your physician, pathology and laboratory when they apply.

We also accept Care Credit, a convenient, monthly payment program for yourself and your family specifically designed to pay for healthcare expenses not covered by insurance including co-payments, deductibles and elective treatment and care. Read more about CareCredit payment plans.

PAYMENT FREQUENTY ASKED QUESTIONS

When do I become responsible for my bill? You are legally responsible for your bill at the time you receive services. The Surgery Center requires all patient balances be paid, or acceptable payment arrangements made, upon receipt of your bill. As stated on our statements, we will bill third party liability insurance for you if requested.

How will I know what portion of the bill I should pay? The amount you owe can be found on your bill. Your bill should identify the total charges, the amount submitted to insurance, and the amount you owe. If insurance has paid part of your claim, the statement will identify the amount paid by insurance and the amount you owe. The explanation of benefits from your insurance company will also indicate which charges you are responsible for.

What forms of payment do you accept?

If payment in full is not possible, you can make payment arrangements by contacting Patient Financial Services at (863) 682-3239.

What is a deductible or co-payment? The deductible of a health insurance plan is the dollar amount that you must pay before your coverage kicks in. So if you had a deductible of $500, you would have to pay $500 of your medical bills before insurance started picking up the tab. The deductible typically resets on Jan 1 of each year. After you meet your deductible, you may also have a co-insurance or a percentage of patient responsibility.

A co-insurance is the percentage of patient responsibility, the most common plans are 90/10 plans, 80/20, 85/15 and 70/30. In these plans the insurance company pays the larger percentage and the patient pays the smaller portion.

A co-payment is a flat amount paid for each visit to a provider. If you have a $50 outpatient co-payment, you must pay $50 for each visit and your insurance company will pay for the remaining balance on all covered services. Co-pay’s may vary depending on the location of the visit (i.e. urgent care, emergency department, outpatient hospital clinic and/or facility).

Do I have to pay my co-payment at the time of service? Yes. You are expected to pay your estimated co-payment when services are provided.

What will I owe after insurance has paid? Insurance contracts vary a great deal depending on allowed services, co-payment amounts, deductibles, and co-insurance. Because of this, it is impossible to know exactly how much your insurance company will pay or how much you will have to pay.
